  • Parent Consent for Children to Use Digital Resources: We need parent consent to use many of our excellent online tools and website when we teach your children.   Click here to sign for your student(s).

  • Student Rights & Responsibilities:  please review the Student Rights and Responsibilities and related resources at www.fcps.edu/srr
    • After reviewing the information, fill out the SR&R Parent Acknowledgement Form using this link we have provided.  The form must be filled out one time per student.

  • Family Life Education (FLE) Opt-Out Forms
    • Optional: If you wish to opt your child out of all or some of the Family Life Education lessons, please complete this form and return it prior to FLE instruction. These forms are available by grade level and in multiple languages.  
  • Annual Notice of Survey, Records, Curriculum, Privacy, and Related Opt-Out Forms (Optional)
    • If you wish to opt your child out of any activities, you must complete and return the appropriate forms to your child's school. Click here for the Forms and more information.
